  • Submit:
    • An Application for Licensure by Examination
    • Completed fingerprint cards
    • A recent 2′ by 2′ passport photo
    • A completed Request for Accommodation of Disabilities form (if necessary)
    • Completed request for transcript forms and nursing school transcripts sent directly from the school that individual graduated

Becoming an RN by Endorsement in California

In order to become an RN by endorsement in California, an individual must:

  • Pay the required fees
  • Submit:
    • An Application for Licensure by Endorsement including a US social security number
    • Completed fingerprint cards
    • A recent 2′ by 2′ passport photo
    • Completed request for transcript forms and nursing school transcripts sent directly from the school that individual graduated
  • Be licensed as an RN in another US state
