For consideration for the Practical Nurse program, the following items are required:
-Applicant must be 18 years of age or older.
-High School diploma or GED. If high school diploma is from another country, a high school equivalency evaluation must be completed (visit www.naces.org for a list of the approved agencies).
-High School Transcript (and college transcript if you have college credits).
-Applicant must be a Pennsylvania resident (check with the school Director if you are not a Pennsylvania resident).
-Satisfactory score on the entrance exam. Charge is $50 for the application (preferably paid on the website) and $40 for each exam attempt, to be paid by cash or money order (application and testing fees are non-refundable).
-Physical Checkup including physical and mental examination (all forms are provided).
-Pennsylvania Child Abuse History Clearance (forms are available at PITC).
-Current CPR for Healthcare Providers card.
-Personal interview with three references.
-Criminal History Check (prohibitive offences listed in act 169 of 1996 should be excluded).
